IAFP Selects Health Directions as Consultant For PIN Pilot

Chicago, Ill. — The Illinois Academy of Family Physicians (IAFP) has selected Health Directions, a Chicago-based national healthcare consulting firm, to coach its members’ practices for a Practice Improvement Network (PIN) small practice pilot project. The PIN is a two-year commitment by IAFP to provide members with the expertise and resources to implement medical home strategies.

“We want our members to be ready when medical homes become mainstream,” said Vincent D. Keenan, CAE, IAFP Executive Vice President. “Health Directions has the expertise to help our members capture new revenue opportunities as they work toward reaching performance measures such as Meaningful Use and medical home designation.”

The small practice pilot program is limited to 30 total practices in Illinois with eight or fewer primary care providers in each practice, with practices in both urban and rural areas throughout the state of Illinois. IAFP will match small practice investment dollars up to $5,000 over an 18-month period.

“We look forward to working with IAFP and its member physicians to provide them with the knowledge and tools they need to help advance family medicine for the future,” said Daniel J. Marino, Health Directions President and CEO. “Right now physicians are inundated with information on medical homes and accountable care and we are able to sift through the noise and help them focus on the incremental changes they can make to deliver patient-centered care.”

Health Directions will work with the IAFP member in reaching PIN goals:

  • Coordinated and comprehensive care with a team-based approach
  • Patient safety
  • Payment reform that rewards primary care
  • Eliminate duplication and waste
  • Measureable reported improved outcomes
  • Patient records available at the point of care
  • Lower the cost burden tied to preventable illness and chronic diseases
  • Timely preventive care services
  • Reduced disparities
  • Access to primary care
  • A personal physician for all patients
  • Patient and family education partnership

Health Directions has been retained through year-end 2012 to work with IAFP and its members.
